• An Allied Care Coach provides support and guidance to individuals within the healthcare field, often assisting with patient care coordination, health management, and navigating the healthcare system.
• They may work with patients, families, or other healthcare professionals to optimize health outcomes and promote well-being.
• Care Coaches work with patients to improve and facilitate communication about goals, values, and preferences with their loved ones and care teams.
• They discuss goals of care to get a better understanding of what is most important to patients, their wishes, and priorities.

Profile 1 – Graduate of Bachelor’s Degree (with or without license)

Experience Required:

• 1-year direct adult patient care such as but not limited to
• Long term care
• Home and community-based services
• Hospital/facility settings
• Case management and/or working with managed care organizations
• OR
• BPO healthcare – MEMBER services via phone

Not Qualified Experiences:

• Catering to Providers and/or pediatrics only
• Interactions via email/chat only
• Any tasks not directly talking to patients

Profile 2 - Associate Graduate/College Graduate (Non-Allied Courses)

 Experience Required:

2-year direct adult patient care such as but not limited to

• -Long term care
• Home and community-based services
• Hospital/facility settings
• Case management and/or working with managed care organizations
• OR
• BPO healthcare – MEMBER services via phone

Not Qualified Experiences: Catering to providers and/or pediatrics only

• 28,000 basic pay plus 1,000 flex fund allowance
